Our Medical Report Section function is to process application for

  • Medical report
  • Completion of Insurance Form
  • Completion of PERKESO, KWSP & etc.
  • Investigation reports such as Laboratory Result or Radiology Result
  • Copies of Discharge Summary, Referral Letter & etc.

Who Can Apply?

  • Patients who are healthy/mentally fit, and 18 years old and above
  • For patients under 18 years old, or those with a mental incapacity to provide consent, a photocopy of identification documents or legal documents as a proof of relationship and consent are required from the parent or legal representative
  • Next of Kin i.e spouse, children, parent
  • Appointed agent (insurance, legal firm & others)
  • Authorised Government Agency (PDRM, KKM, SPRM & others)
  • Deceased Patient – a photocopy of death certificate, marriage certificate, Letter of Administration as a proof of relationship to the deceased or a grant of probate and consent are required from the legal representative. A nominee letter from the insurance company is required for claim processing

  • The payment can be made via cash, credit card, and bank transfer to UM Specialist Centre. For online transaction, please contact Medical Records Management Department at [email protected] to submit bank transfer receipt.
  • A gentle reminder that all outstanding amounts in the Hospital shall be settled together with the medical report fee to avoid any delay in processing.

Upon the completion of the report, we will inform the applicant via Telephone/E-mail.

The collection can be made based on your preferred mode of collection:-

Collect the report from the counters at Medical Records Management Office:

  • ID verification (NRIC or Passport) is required before collection
  • If collection is on behalf of the patient, an authorisation letter signed by the patient is required

Receive the report via email:

  • Online payment done and a scanned copy of bank transfer receipt is emailed to [email protected]
  • Report will be sent to the email address provided in the Application Form
  • The documents will be encrypted with a password for security purposes

Receive the report via postage:

  • Online payment done and a scanned copy of bank transfer receipt is emailed to [email protected]
  • Report will be couriered to the mailing address provided

1. How to apply for Medical Report?

  • Walk in to Medical Report Counter @ GF, Counter 13, UMSC Building.
  • Email to [email protected]
  • Post to Medical Records Department, UM Specialist Centre (UMSC), Lot 28, Lorong Universiti, Lembah Pantai, 50603 Kuala Lumpur, Malaysia.

2. When do I submit the form?

After completion of your visit to the Outpatient Clinic or after discharge from the ward.

3. Is there any payment or fee for Medical Report?

Payments are subject to the type of application. Please refer to Medical Report Team at +603 7841 4012 or email [email protected] for further details.

4. What is mode of payment for Medical Report?

  • Cash
  • Online Transaction – account detail (Please submit the scanned copy of bank transfer receipt via email to [email protected])
  • Debit or Credit Card

5. Who can apply for a Medical Report?

  • A Patient aged 18 years and above.
  • A parent or legal guardian can apply for patient who is a minor (17 years old and below).
  • Interested third parties (i.e. insurance company and employer) with patient’s consent.
  • Next of Kin of disabled / deceased patient’s. Please refer UMSC List of Immediate Next of Kin.

UMSC list of Immediate Next of Kin for disabled (unable to give voluntary consent) or deceased patient.

The Next of Kin could be any one of the following who is available:

    • the patient’s spouse;
    • the patient’s child
    • the patient’s parent
    • the patient’s sibling
    • the patient’s grandparent
    • the patient’s grandchild
    • an adult who is an immediate relative to the patient by marriage

The criteria for Immediate Next of Kin must:

    • be at least 18 years of age (adult)
    • have been in contact with the patient during the preceding 12 months
    • have no dispute with the patient
    • be willing to comply with the duties
    • not misuse the patient’s record

6. I’m a family member of the patient but not allow to request the medical report. Why?

We only allow the request from Next of Kin as registered in our system.

7. Can the family member or insurance agent or representative request and collect the report on my behalf?

Yes, as provided in the application form.

8. Can I request for a copy of investigation result?

Yes. You will be receiving a copy of investigation result within a week from the date apply.

9. How long does it take for the doctor to complete my medical report?

  • The timeline for Report Completion is within 3 – 6 weeks from the date we receive your complete request. Delay in medical report completion may be the result of:
    • The patient has upcoming clinic appointments
    • The patient has been hospitalised after submitting the application
    • The patient’s file being used by other departments (for various reasons)
    • Patient has multiple reports requested from several doctors
    • The doctor is away on leave.

10. Will Medical Report Unit provide all the necessary forms for me to claim? i.e. insurance claim form, PERKESO, EPF etc.

No. Medical Report Unit will not provide any form. You should request for the form(s)to be completed by our doctor from respective organisations. We will not be held responsible if you sent us the wrong form(s).

11. How will I receive the Completed Medical Report?

You can collect the report personally at Medical Report Counter or request the report to be posted at the address you have provided.

12. If I wish to cancel the application, can I get the refund?

– Yes, we allowed for refund but only for the medical report fee and postage (if any).
– The administration fee will not refund back as we already processing your application.

13. What will happen if the application form submitted there is an attempt to cheat?

During verification process, if there any discrepancy was found, we will on hold/reject the application and the payment will not be refund back to requestor.

14. My last visit to UMSC was 10 years ago. Do I need an appointment with a doctor to get the latest medical report?

– Yes, you should get the latest appointment so that the doctor can assess the latest condition and can advise what is better for you. After that, you can request for latest medical report.
– According to UMSC policy, if patient didn’t come to UMSC after 7 years from last visit, the file will be disposed.

15. I need a simple letter from my doctor confirming that I am UMSC’s patient, can you explain the process?

First, we need to know the purpose of the letter. (If the request for comprehensive detail, we advise to apply for medical report).

16. Can a relative ask a question regarding the patient’s wellbeing?

No. only can be release with patient consent.

17. I am a Government Employee/Pensioner, is my medical report charge covered by Government?

No, it is not covered by the Government.

18. I misplaced my MC slip during discharged, can I get a copy – Certified True Copy (CTC)?

There will be charges of RM10 for MC copy.
